What to Bring to Your First Appointment
Please bring an up-to-date photo ID and your current insurance card to your first visit. We also ask that you arrive 15 minutes early to complete new patient paperwork, including HIPAA forms, consent to treat, and billing agreements.

Future Online Services
Starting January 2025, we will enhance convenience by offering many of our services online through the Athena Patient Portal. This secure platform allows patients to send and receive HIPAA-compliant information, access medical records, and communicate with our staff and providers from the comfort of home.

Scheduling Appointments
We believe in the personal touch of speaking directly with you to schedule your appointments. Please call our clinic to talk with a team member who will take the time to understand your medical needs and ensure you’re scheduled appropriately. Our staff can also consult directly with medical providers, if necessary, to address urgent concerns.

• Acute care visits: Often available same-day or next-day.
